We’re looking for a Senior Social Worker to join a forward-thinking Learning Disability & Autism Intake Team in the North East. In this role, you’ll lead by example in delivering proactive, person-centred care that puts the needs and dignity of every individual first. You'll work alongside a collaborative, multi-disciplinary team that values innovation, reflective practice, and strengths-based approaches. 💼 Your Key Responsibilities:

Conduct strength-based assessments to identify social care needs and appropriate interventions.
Develop and review personalised care and support plans in collaboration with individuals and families.
Make informed decisions using your knowledge of legislation, risk assessment, and safeguarding practices.
Signpost service users to appropriate services, resources, and community support.
Provide advice and support to unqualified staff and students.
Work flexibly and collaboratively with health, housing, and voluntary sector partners.
Promote dignity, empowerment, and independence in every interaction.

✅ About You:

Qualified Social Worker registered with Social Work England.
Minimum of 4 years' post-qualifying experience in Adult Social Care.
Strong knowledge of current social care legislation, including the Care Act 2014 and Mental Capacity Act.
Experience supporting adults with learning disabilities and/or autism.
Skilled in managing complex caseloads while maintaining high standards of practice.
Able to build strong relationships with individuals, carers, and partner agencies.
Proactive, reflective, and committed to continuous improvement.
