To work with adults in North Lincolnshire Job Title: Occupational Therapist – Adult Social Care (Community-Based) 
Location: Various community settings 
Hours: Full-time 
About the Role: 
We are seeking a dedicated and compassionate Occupational Therapist to join our Adult Social Care team, working within the community to support adults with a wide range of needs. This is a rewarding opportunity to make a real difference in people’s lives by promoting independence, wellbeing, and safety in their own homes and communities. 
Key Responsibilities: 
To complete holistic assessments of adults with physical, sensory, and cognitive impairments. 
Develop and implement person-centred intervention plans to promote independence and reduce risk. 
Recommend and facilitate the provision of equipment, adaptations, and assistive technology  
Work collaboratively with people of North Lincolnshire, carers, families, and multi-disciplinary teams. 
Maintain accurate and timely records in line with professional and legal standards. 
Contribute to safeguarding processes and risk management strategies. 
Promote the principles of the Care Act, Mental Capacity Act, and other relevant legislation. 
£37 per hour
